GLP-1 weight loss: what a nurse practitioner actually monitors
The medication is the easy part. What decides whether you keep the result is what happens around it — and that is the part telehealth apps skip.

What varies enormously is what happens around them — whether anyone checks that the weight coming off is fat rather than muscle, whether side effects are managed or endured, and whether there is a plan for what happens when you stop.
How it starts
A full medical intake first: history, current medications, previous attempts, and labs where indicated. This is where a supervised programme differs from an app that mails you a pen — some people should not be on these medications at all, and that has to be established by someone qualified to establish it.

The part that decides whether you keep it
Protein
Appetite suppression means you will eat less of everything, including protein — and protein is what protects muscle during a deficit. Protein first at every meal is the single most important habit on this medication, and most people need a concrete target rather than general advice.
Resistance training
Twice a week, non-negotiable. Weight lost as muscle lowers your metabolic rate, which is precisely the mechanism that makes regain easier afterwards. This is why the scale alone is a poor measure and body composition is tracked instead.
Hydration
Two to three litres a day. It reduces nausea substantially and prevents the constipation that otherwise becomes the main complaint by week three.
What happens to your face
Rapid loss deflates the mid-face — the effect the press labelled "Ozempic face". It is real, and it is simply fat loss in a place you did not want it. It is also entirely addressable: a biostimulator such as Sculptra, or radiofrequency for the laxity that follows, restores structure a few months in. Being warned about it in advance is part of a proper plan.
How it ends
Many people taper off once weight and body composition have stabilised and the habits are genuinely established. Some stay on a low maintenance dose. Neither is a failure — but the plan should exist from month one, rather than being improvised when the prescription runs out.
Cost
The consultation and monthly medical visits are $50. Medication is prescribed separately and priced by pharmacy and dose, which varies more than most people expect — it is worth pricing two or three pharmacies. Lipolytic injections for localised fat are $75, and Kybella for the double chin is $600.
Common questions
Will I regain the weight when I stop?
Only if nothing else changed. Regain is common when the medication was the entire strategy — which is why protein, training and body composition are tracked from week one rather than mentioned at the end.
How fast should I be losing?
Roughly one to two pounds a week is the range that preserves muscle. Faster than that is usually costing you lean tissue and is worth slowing deliberately.
Do I need labs first?
In most cases yes, and always if you have relevant history. That assessment is part of the intake.
Can I get this without seeing anyone in person?
Elsewhere, yes. Here, no — the point of a supervised programme is that someone qualified is actually looking at you each month.
