Masseter Botox: jaw slimming, grinding, and what it really does

Two entirely different reasons people ask for it — grinding and jaw width — and the timeline is not the same for either.

The masseter is the muscle at the angle of your jaw — the one that bulges when you clench. It is one of the strongest muscles in the body relative to its size, and treating it does two useful things at once: it stops the damage from grinding, and it narrows the lower face.

Reason one: grinding and jaw pain

Bruxism wears enamel, cracks teeth, and produces morning headaches and jaw ache that people often blame on stress alone. Relaxing the masseter does not stop the brain from sending the clench signal, but it removes most of the force behind it. Patients typically report the morning headache going first, within two to three weeks.

Reason two: a softer, narrower lower face

A hypertrophied masseter squares the jaw. Years of clenching, or simply genetics, build the muscle the way any repeated load would. Reducing it slims the lower third of the face and produces a more tapered outline — which is why the treatment is far more requested for the aesthetic effect than the dental one.

The timeline is slower than facial Botox

This is the point most often misunderstood. Facial Botox shows in days. Masseter slimming is a muscle-atrophy effect and takes months. Judging it at two weeks and concluding it did not work is the most common mistake.

Dosing and cost

Typically 40 to 60 units in total, split between both sides — considerably more than an entire upper face, because the muscle is large and strong. At $12 a unit that is $480 to $720; the clinic price for the treatment is $400 when dosing sits in the usual range.

After two or three well-spaced rounds, many people find the muscle stays smaller for longer and the interval between treatments stretches out. That is a genuine saving that only appears from the second year onward.

The trade-offs nobody mentions

  • Chewing tough food feels weaker for the first few weeks — steak and bagels are the usual complaints
  • Over-dosing or poor placement can flatten the cheek contour or, rarely, affect the smile
  • If you have significant facial fat rather than muscle bulk, slimming will underdeliver — this treats muscle, not fat
  • Stopping means the muscle gradually rebuilds; nothing is damaged, but the effect is not permanent

Common questions

Will my face look sunken?

Not at correct dosing. Excessive treatment over years can hollow the area, which is one reason doses are reviewed each round rather than repeated automatically.

How soon can I stop wearing my night guard?

You shouldn't, unless your dentist says so. The treatment reduces force, but the guard protects against contact.

Can I combine it with jawline filler?

Yes, and it is a common pairing: masseter Botox narrows the width, filler or Radiesse defines the border. Slimming first, then define.

Is it covered by insurance for bruxism?

Occasionally, with documentation from a dentist. It is worth asking, but plan on paying out of pocket.
